If you prefer manual registration, please download the form here and send the filled up form to Ms.Suzainur via email at email@example.com. Once the secretariat receives the form, an invoice on the workshop fee will be issued and emailed. Registered participants will be required to make remittance of payments via Electronic Fund Transfer (EFT) ONLY with the bank account details as indicated on the invoice. All the payment are to be made BEFORE 19th October 2012.
CANCELLATION AND REFUND POLICY
Notification of cancellation must be made in writing to firstname.lastname@example.org. The following provisions will take effect:
- No refund will be given for any cancellations after 19th October 2012.
- The registration is non-transferable.
