UNU IIGH hosted two expert panel meetings on 9 and 16 April 2012 to discuss the care pathway for post stroke patients after discharge from tertairy care.
The meetings were related to the PhD thesis project on the Development of Integrated Care Pathway for Post Stroke (iCAPPS) patients residing at home in the Malaysian community (Research ID: UKM-GUP-2011-327).
The meetings were attended by Family Medicine Specialists, Neurologists, Rehabilitation Physicians, Therapists and Nurses from both public and university-based healthcare facilities. It is believed that this was the first meeting in Malaysia which attempted to coordinate the care of post stroke patients within existing stroke care providers in public healthcare centres.
Findings of earlier pilot studies done among Family Medicine Specialists in public service and analysis of stroke patients managed at a primary care facility were used to identify gaps in healthcare delivery.
